Even after chemotherapy dose adjustment

Vienna, Austria: Women who are overweight or obese when they are diagnosed with breast cancer are at higher risk of cancer recurrence or related death than are leaner women, according to a new study to be presented to the 8th European Breast Cancer Conference (EBCC-8) today (Friday). This finding held true even though the study mandated that chemotherapy dosage be adjusted for body weight, and adds further to the evidence that lifestyle factors can influence cancer prognosis, a researcher will tell the conference.

Dr. Jennifer Ligibel, a medical oncologist at the Dana-Farber Cancer Institute, Boston, USA, and an Associate Professor at Harvard Medical School, and colleagues, studied data from 1909 patients who were enrolled into a study called CALGB 9741 between 1997 and 1999. The study was set up to investigate different dosing schedules for adjuvant chemotherapy in patients where cancer cells were found in the lymph nodes (node-positive cancer). The presence of such cells in the lymph nodes means that there is a higher chance of cancer returning after surgery.

After extracting height and weight data from the patient records, they went on to evaluate the relationship between body mass index (BMI) with relapse-free survival (RFS) and overall survival (OS). 1.2% of the patients were underweight, 32.6% normal weight, 32.9% overweight, and 33.3% obese. 49% of patients were menopausal, 65% had oestrogen-receptor positive cancers, where the presence of oestrogen encourages the tumour to grow, and 70% received the oestrogen-receptor blocking treatment, tamoxifen.

"Several other studies have shown that being overweight or obese at the time that a woman is diagnosed with breast cancer is linked to a higher risk of recurrence. However, questions have been raised in the past whether obese women were receiving relatively lower doses of chemotherapy due to their weight. Our study mandated that each patient received a chemotherapy dose adjusted to her weight, so these results suggest that treatment factors are not responsible for the differences in recurrence rates seen in heavier women," Dr. Ligibel will say. "We found that BMI was related to both RFS and OS; for example, the ten-year RFS of a patient who was overweight was 70%; compared with 65% for one who was obese."

Although the link between obesity and the development of breast cancer is well known, there has been less research to date looking at its effect on cancer recurrence and survival.

"When you consider that data from 2007-8 [1] show that 68% of US adults aged 20 years and over were overweight or obese, as compared to only 56% of the same group in 1998-1994, you can see the way the problem is growing. That is why we think it is a matter of urgency to find out as much about the relationship between obesity and cancer as we can," Dr. Ligibel will say.

The researchers intend to follow up their work by learning more about how weight-related factors could influence breast cancer outcomes. A number of studies currently underway are looking at how changing lifestyle behaviours, for example losing weight, exercising more, and eating a better diet, affects the hormones in women's bodies that have been linked with breast cancer. Ultimately, they say, they are interested in studying the impact of deliberate weight loss on the risk of recurrence in women with early breast cancer.

"Obesity is a modifiable factor, and although there is not yet enough evidence to say with certainty that losing weight or exercising more regularly will decrease the risk of breast cancer recurrence, there are consistent links between lifestyle factors such as diet, weight and physical activity patterns and breast cancer prognosis. If future studies show that making changes in lifestyle behaviours for women with early breast cancer will improve survival rates, then lifestyle interventions may one day become a standard part of breast cancer care," Dr. Ligibel will conclude.

Professor David Cameron, from the University of Edinburgh (Edinburgh, UK) and chair of EBCC-8 said: "Whilst these are important findings for women with breast cancer, we need to recognise that the reason overweight women have poorer outcomes is not clear. There are a lot of health reasons why overweight women should try and get back to a normal weight, but this is not always easy, and as the authors acknowledge, we don't yet know that losing weight after a breast cancer diagnosis will make a difference."

Swiss probe deadly bus crash as families arrive

SION, Switzerland/HEVERLEE, Belgium (Reuters) - Distraught parents flew to Switzerland on Wednesday after a bus carrying a Belgian school group home from a ski trip crashed into the wall of a Swiss tunnel, killing 22 children and six others.

Twenty-four passengers remained in hospital, including three children in Lausanne with critical injuries, but the other survivors were out of danger, Swiss officials said.

Swiss President Evelyn Widmer-Schlumpf and Belgian Prime Minister Elio Di Rupo, speaking at a news conference in the town of Sion near the crash site, paid tribute to the victims and the 200 rescue workers who pulled the injured from the wreckage after the bus rammed into a wall inside a tunnel on Tuesday night.

"When a drama like this happens, when we lose a child or have a child suffering in hospital, there are no words. It is important to console the families," Di Rupo said.

Twenty-one of the dead were Belgian nationals and seven were Dutch, according to Swiss officials. The Dutch Foreign Ministry said three Dutch children in the bus were injured. Most children aboard were aged about 12.

The man in charge of ambulances at the tunnel, Alain Rittiner, described a scene of horror.

"Access to the vehicle, which is quite high, was difficult and it was hard to remove the victims. Emotions ran high, and with so many children, it was an absolute horror," he told Swiss television.

A police photograph showed the bus had smashed into the side of a tunnel, with the front ripped open, broken glass and debris strewn on the road and rescue workers climbing in through side windows. It was later towed away from the scene.

About 200 police, firefighters, doctors and medics worked through the night at the scene, while 12 ambulances and eight helicopters took the injured to hospitals in the holiday region.

Belgium plans to hold a national day of mourning. A memorial mass was set for Thursday in the Swiss town of Sierre. Widmer-Schlumpf, a mother of three, said Switzerland was doing everything to support victims and their families.

Olivier Elsig, prosecutor for Valais canton (state), said that video surveillance images from the tunnel, where the speed limit is 100 kmh (62 mph), showed no other vehicle was involved in the accident and the road was dry and in "good condition".

"The bus did not appear to be travelling too fast," Elsig told the news conference. "I immediately ordered an autopsy of the deceased driver, which is under way at this very moment."

In Washington, a spokesman for the National Security Council said U.S. President Barack Obama "sends his deepest condolences to the victims and their families of last night's bus accident in Switzerland. The loss of so many young lives is especially heartbreaking".

VIOLENT CRASH

It was the worst accident in Switzerland since 1982, when 39 German tourists were killed on a railway crossing after a train hit their bus.

The bus had travelled only 15-20 km from the Swiss ski resort of Val d'Anniviers before entering the tunnel. "The children were all wearing seat belts but the shock of the crash was violent," Elsig said.

There were three possible causes for the crash, he said: a technical problem; the driver may have become ill; or human error.

About 100 family members, who flew to Geneva from Belgium, were taken by buses to the Valais canton. Some began visiting injured children in Sion hospital, while others were being counseled by psychologists in crisis groups.

A mortuary was set up and bodies were being identified. So far, 22 of the 24 injured had been identified, officials said.

Gathering at primary schools in Belgium before boarding a military aircraft for Switzerland, some of the parents did not know whether their children were dead or alive.

"There's no news, simply no news," said one red-eyed father.

Of the 24 injured, three were in a coma. "Three children are potentially gravely injured and we hope that their condition stabilizes, but we must remain guarded," Jean-Pierre Deslarzes, a doctor heading the rescue operation, told the news conference.

Most pupils were from the towns of Lommel and Heverlee in Belgium's Dutch-speaking Flanders region.

Children at St Lambertus school in Heverlee, a suburb of Leuven, were informed about the accident at an assembly before classes. Flowers were laid outside the Catholic school where eight children were still unaccounted for.

A teacher from St Lambertus were killed along with the bus's two drivers and three other adults.

Teachers covered boards outside the school and the school gates with their pupils' drawings - many rainbows, flowers and references to "Mister Frank", the sixth-grade teacher who died.

"I hope we will wake up soon and that this nightmare will be over," one pupil wrote.

The bus had not been driving for long after heading down from the resort and had only been on the valley highway towards the Swiss town of Sierre for 2 km (1.2 miles) when it hit the curb and veered head-on into an emergency siding in a tunnel.

The bus was owned by Belgian company Toptours, which Belgian Transport Minister Melchior Wathlelet said had a solid reputation. The driver had arrived at the resort a day before the trip, according to the rules, and the bus had passed a mechanic's test five months ago.

Of the 10 Dutch children on the bus, nine lived in Belgium and one in the Netherlands. There was also one German and a Polish person on the bus, the Belgian government said.

Rutgers webcam spying trial is heading to jury

NEW BRUNSWICK, N.J. (AP) ? After nearly two weeks of testimony, the case that kick-started national conversations about gay youths and Internet privacy is going to a jury that must decide whether Dharun Ravi is a criminal or just a young man who was confused by seeing two men kiss.

Ravi, now 20, is accused of viewing a few seconds of his roommate's encounter with another man in their dorm room at Rutgers University and telling people about it in text messages, tweets and in person. He could face years in prison if convicted of charges including invasion of privacy and bias intimidation, a hate crime.

Lawyers gave their summations Tuesday in the case, which has gotten enormous attention since the events of September 2010, when the roommate, Tyler Clementi, jumped to his death from the George Washington Bridge.

The trial, which included testimony from about 30 witnesses over 12 days in addition to the closing arguments, focused on a few days in the dorm where Ravi and Clementi, both 18-year-olds from well-off New Jersey suburbs, were randomly assigned to be roommates for their first year at Rutgers.

Defense attorney Steven Altman told jurors that Ravi was surprised to turn on his webcam and see his roommate in an intimate situation with another man. He emphasized that there was no recording, no broadcast and no YouTube video of the Sept. 19, 2010, encounter.

And he said Ravi was not acting out of hatred of his roommate or gays in general when he saw the image from his webcam on the computer of another student.

"If there's hate in Dharun's heart, if there's ugliness in Dharun's heart, where's there some information and some evidence to support it?" Altman asked jurors.

Ravi tweeted and talked about what he saw, but Altman said he was only doing so because he was young, had never before seen men kissing and did not know what to do. He'd turned on the webcam in the first place, Altman said, because he was worried about what was happening in his room after seeing Clementi's guest, whom Ravi described as "older" and "sketchy."

His client, Altman said, was concerned about whether the stranger might take the iPad he'd left in the room.

Julia McClure, a prosecutor for Middlesex County, reminded jurors of testimony from some of Ravi's high school friends that even before Ravi moved into the dorm, he was concerned about having a gay roommate.

"He was so shocked that within about four minutes, he sent out a tweet, because he was seeking advice?" McClure asked. And, she said, there was evidence that he then told other students about what he'd seen and invited them to a friend's room where they could see for themselves.

The challenge for jurors could be deciding whether the laws apply to what Ravi is alleged to have done.

He faces 15 charges. Four are invasion of privacy and attempted invasion of privacy charges, where the required proof is that he saw or disseminated images ? or attempted to ? of private parts or sex acts, or a situation where someone might reasonably expect to see them.

Four charges allege bias intimidation. Ravi can be convicted of intimidation if he's also found guilty of an underlying invasion-of-privacy charge. Two of those charges are second-degree crimes punishable by up to 10 years in prison ? the most significant penalties he faces if convicted.

Seven charges accuse him of trying to cover his tracks. Among the allegations: that he deleted and changed Twitter postings and text messages and told another witness what to say.

Clementi's death was one in a string of suicides by young gays around the country in September 2010 and became probably the best known. President Barack Obama commented on it in an online video, as did talk show host Ellen DeGeneres.

New Jersey lawmakers hastened passage of an anti-bullying law because of the case, and Rutgers changed housing policies to allow opposite-sex roommates in an effort to make a more comfortable environment for lesbian, gay, bisexual and transgender students.

Colombia sends suspect in singer slay to Guatemala

BOGOTA, Colombia (AP) ? A Costa Rican man who authorities say may have links to Mexico's powerful Sinaloa drug cartel was expelled to Guatemala in connection with last year's fatal shooting of Argentine folk singer Facundo Cabral.

Costa Rican authorities had asked Colombia to halt the deportation of Fernando Alejandro Jimenez Gonzalez because the suspect could face execution in Guatemala and Costa Rica does not accept the application of the death penalty to its citizens.

But Guatemalan Interior Minister Mauricio Lopez Bonilla later said his country had sent Costa Rica a diplomatic note guaranteeing that the death penalty would not be applied, allowing the expulsion to proceed

An official with Colombia's Foreign Ministry who was not authorized to speak to the press told The Associated Press late Tuesday that the plane had finally departed from an anti-drug base in Colombia's capital. Earlier, a police statement had said that Jimenez had already been flown out.

Jimenez arrived at the Guatemala City airport early Wednesday and was taken to National Police headquarters in the capital, said Minister of Government Mauricio Lopez Bonilla.

Jimenez, 38, was arrested Saturday by counternarcotics police in a speedboat that Colombia's navy had tracked from Panama into Colombian territorial waters, said Colombia's national police chief, Gen. Oscar Naranjo.

Costa Rican authorities have said Jimenez is wanted at home on suspicion of money laundering and drug trafficking, but they asked Colombia to send him directly to Guatemala for the Cabral case.

The Colombian police said in a communique that according to information gathered by Costa Rican security agencies that Jimenez could be a link between the Sinaloa cartel headed by Mexican druglord Joaquin "El Chapo" Guzman and Colombian narco brothers Luis Enrique and Javier Antonio Calle Serna.

Naranjo said Colombian police believe Jimenez arranged his attempt to enter Colombia on Saturday with the help of the brothers.

Costa Rican officials allege Jimenez is head of a criminal group that launders money in Central America, and they believe Cabral was the unintended victim of an attack on another man in the same car related to a rift over stolen drug money. Drug cartels, some from Mexico, have established themselves in Central America and grow and ship drugs in the region.

Cabral, 74, was shot to death July 9 after giving a concert in Quetzaltenango, a Guatemalan city 200 kilometers (120 miles) west of the capital, Guatemala City.

Authorities believe the attack was aimed at Cabral's promoter, Nicaraguan businessman Henry Farina, who was driving the singer to the airport. Farina was wounded when their car was ambushed by assailants in three cars.

Guatemalan officials have said the killers appeared to have had Farina under surveillance for about a week before the attack and didn't know Cabral was in the vehicle.

Three other suspects were arrested last year in Guatemala, but Jimenez had remained a fugitive.

Cabral rose to fame in the early 1970s as one of a generation of singers who mixed political protest with literary lyrics. He created deep bonds with an audience struggling through an era of revolution and repression across Latin America.

(AP) ? We were robbed! That's how many Peruvians feel, now that U.S. courts have given Spain the 17 tons of silver and gold coins that a private company salvaged from the wreck of a colonial-era Spanish sailing ship.

The treasure's origin is not in dispute. The metals were mined and the coins minted in the Andes. The Spanish navy frigate that was carrying them to Spain exploded during an attack by British warships in 1804.

Peru argued it should get the precious metal recovered from the Nuestra Senora de Las Mercedes. But its legal case was sunk in large part by a historical fact: This country was, at the time, a Spanish dependency. It didn't gain independence until 1821, the last bastion of Spanish rule in South America.

"It is uncontested that the Mercedes is the property of Spain," a three-judge U.S. appeals court ruled in September.

Many Peruvians, however, feel they are entitled to the booty because of colonial Spain's violent, exploitative legacy. Countless natives of the Andes were forced to abandon home and family and toil in life-choking conditions extracting ore underground.

"Spain's progenitors were genocidal to our progenitors, the indigenous of Peru, thousands if not millions of whom died in underground mines going after that metal," said Rodolfo Rojas Villanueva, an activist with the eco-cultural movement Patria Verde.

Other Peruvians would be happy to get a share of the 594,000 coins, whose value has been estimated at $500 million, not so much as reparations but because they are Peru's heritage.

Spanish officials flatly reject any Peruvian claim.

Spain's culture minister, Jose Ignacio Wert, received the treasure with considerable fanfare Feb. 27 after the U.S. Supreme Court rejected a bid by Peru to halt the shipment. Wert said U.S. courts were clear: "The legacy of the Mercedes belongs to Spain."

The coins, mostly silver reals but also gold doubloons, came from ore mined in present-day Peru and Bolivia and likely also Colombia and Chile. It's not clear exactly what portion was minted in Lima, Spain's continental capital after its conquistadors subjugated the Incas.

Odyssey Marine Exploration of Tampa, Florida, recovered the treasure in 2007 about 160 kilometers (100 miles) west of the Strait of Gibraltar and placed it in the custody of U.S. courts, which declared the find exempt from their jurisdiction and ordered it turned over to Spain.

Peru and Odyssey have appeals before the U.S. Supreme Court seeking to overturn those rulings.

Peru's government says the coins are the country's patrimony.

"There existed an entity, a country that had not yet become independent but was a territory that later converted itself into an independent country, that is called Peru," said Jose Antonio Garcia Belaunde, foreign minister in the 2006-2011 government of President Alan Garcia. "The money belonged to that territory."

Peru's ambassador in Washington, Harold Forsyth, put it less abstractly: "The ship departed from the port of Callao (adjacent to Lima) with a cargo of coins minted in Peru, extracted from Peruvian mines with arms and sweat of Peruvians."

Peru has fought previously for archaeological artifacts lost to the developed world. Under Garcia, it successfully campaigned to persuade Yale University to agree to return hundreds of items taken from the famed Inca citadel of Machu Picchu a century ago by the U.S. explorer Hiram Bingham.

In the case of Las Mercedes, it is not just Odyssey and Peru laying claim to the doubloons and reals.

Others include descendants of the ship's captain, Diego de Alvear Ponce de Leon, and of merchants who Odyssey says collectively owned three-quarters of the coins. Those merchants paid Spain a 1 percent conveyance tax.

"In essence, this is an expropriation," said Rafael Fernandez de Lavalle, a Colombian who claims about 800 silver coins and a small chest from Las Mercedes. "It is really upsetting that they can rob you in such a brazen manner."

He is descended from one of the merchants, a Peruvian-born count, Jose Antonio de Lavalle y Cortes, who exported cacao to Spain, and belongs to a group of 17 mostly Peruvian families who have also appealed to the U.S. Supreme Court.

Spanish historians generally defend their country's right to the coins. But some think the nations of origin should be able to display some of it.

"It would be very good to send a part, if only as a loan, as a message of fraternity," said Marina Alfonso, a historian at UNED University in Seville.

Wert, Spain's culture minister, said his nation has not ruled out allowing some of the coins to go on display in Latin America, but stresses it would only be on loan.

Alfonso said most of the recovered silver almost certainly came from Potosi, part of present-day Bolivia and once the world's richest silver lode.

Bolivia's culture minister, Pablo Groux, said numismatic experts have determined the coins were minted in four places: Lima; Popayan, Colombia; Santiago, Chile; and Potosi.

He said his country has formally requested a share of the treasure but decided not to litigate, considering its case weak.

"We think the strategy should be diplomatic," he said.
